If your student already has a standard CLEVNET library card from any participating library, please note the library name and card number when filling out the form below

Opportunity Card Policy

This student Opportunity Card will allow juvenile patrons (ages 0-17) who do not have a parent or guardian present at the time of application to be given a card that limits their borrowing to 3 books at a time.

No other materials will be able to be checked out on this card. No more than 3 books can be borrowed on the card until some or all of the previous items have been returned.

A parent or guardian may upgrade their child’s card when the proper identification is presented at the circulation desk.
